Recruiting Now for PY 2027
CMS has released the LEAD Model RFA. Pathways is submitting an abbreviated application this Summer 2026 and recruiting Participant Practices and Preferred Providers throughout 2026. The Implementation Period runs Fall 2026, and Performance Year 2027 begins January 1, 2027. Early conversations with Pathways help ensure your practice is positioned for the first cohort.
